• All BHL players registering must be registered & financial within a Club and within Brisbane Hockey Association.

That you have read and understood all information supplied by BHA about Short Term/Temporary Clearance

1. nominated and participated in Brisbane trials according to their age eligibility.
2. not been selected in a Brisbane team.
3. been invited by another centre to play with their Representative team.
4. Is not submitted before the announcement of the age-relevant Brisbane Team/s
5. questions must be answered truthfully.

Note Rule:

11.1.7 Playersselected in BHA teamsshall not be eligible to play for another association if they withdraw 
            from the representative team for which they were selected.
        a. Primary registered players of the Brisbane Hockey Association must in the first instance 
           nominate for the BHA Representative Teams if they wish to compete in any State 
           Championship.
       b. If not selected the players may be released to play for another association, however, the 
           nominee must attend all trials and selection process as required. Failure to attend (without 
           accepted reason) will result in the nominee not being released to another centre until all other 
           players who fulfilled the requirements but were not selected have been offered a position in a 
           team.
       c. Associations requiring players to fill their teams must contact BHA with their requirements, and 
           players not selected for BHA will be asked if they wish to play for that association.
       d. BHA reserves the right to recall players to their teams if withdrawals require it.
       e. Any BHA player who plays for another association will not be recorded as a BHA Representative 
           Player.
        f. Players who did not nominate for a BHA team will not be released to another association unless 
           all players who nominated for BHA have been offered a position in the team.

•  Completion of this form does not guarantee that a Short Term Clearance will be provided.
• For every approve application BHA will manually transfered to a restricted access Hockey Queensland form to be processed and approved/not approved by them.
